We are always accepting new patients, and we accept nearly all insurance (with the exception of MediCal & Kaiser). Please call the main office line to set up your new patient appointment.

Annual Physical Examination

This visit is centered around the physical examination, head to toe. For a woman, it may include a breast examination, pelvic examination and Pap smear (unless these are up to date or done elsewhere). For a man, it may include a rectal examination of the prostate (ask during your appointment if it is right for you). We will also try to update your medical history (family history, surgeries, medications, etc.). If you are due for screening examinations (e.g. colonoscopy, mammogram) or vaccines, we will order them.


Medicare Annual Wellness Visit

If you are on Medicare (at any age), your health plan wants us to review issues of personal functionality and safety. It will involve questions about activity, vision, cognitive function, and home safety. It may also include some discussion about advance directives.

A physical exam is not generally part of this visit, though your doctor/clinician may include some brief elements of physical examination. We will also update your medical/surgical/family history and catch you up to date on health screening and vaccines.

This is also not a thorough review of all chronic medical conditions, but we may touch on them during this visit.

A few things to consider on both:

— Either of the visits above is charged to your insurance and does not incur a co-pay.

— We are limited on time, and usually do not have time to address medical issues (other than the briefest of reviews), a new medical complaint, or to review labs ordered previously.

— If we have the time available once we have completed the essential elements of the Annual Physical Exam (PE) or the Medicare Annual Wellness Visit (AWV) we will try to handle additional medical issues you may have, though we may ask that you schedule a separate appointment for these.

— If we do address additional problems, these are not covered by the bill we submit to your insurance for the PE or AWV. This will be billed separately and may require a co-pay.

SPORTS PHYSICAL’S

Our office offers Sports Physicals. Athletes do not need an appointment, we are available Monday through Thursday and athletes need to be here at 4:00pm. to complete the sports physical form. We are aware that the schools give out a form but we have a more comprehensive one. We will send a copy of our form with the athlete to return to the school.

The charge is $30.00

We encourage you to download and complete our sports physical form and bring it with you.
